Everything starts here. The three questions are what we measure. The four tiers are how deep it has gone.

Every item in the survey carries exactly one question and one tier. That single tagging decision is what lets a seven-minute survey be read two completely different ways — by what you are asking about, or by how far it has travelled.

The columns narrow as you move right. That narrowing is the diagnosis: it shows you exactly where your people stop moving.
